Delivering Specialist Care to NDIS participants

Wound Innovations provides specialist wound and stoma care services for eligible participants under the NDIS. Our service supports individuals with complex or chronic needs, delivering high-quality, clinically appropriate care aligned with NDIS goals to promote independence, wellbeing, and improved quality of life.

Our experienced Wound and Stoma Care Nurses deliver tailored, participant-centred care in the home and community setting. We work closely with participants and their support networks to ensure care is practical, sustainable, and integrated into daily routines.

Through this flexible, participant-focused model, Wound Innovations delivers accessible, coordinated care that supports individual needs and goals.

In-Home Consultations

Clinicians provide comprehensive in-home assessments in a familiar environment, incorporating clinical history, current treatments, environmental factors, and individual goals. We collaborate with participants, families, support workers, and healthcare providers to develop personalised management plans. Where required, additional assessments may be undertaken to support optimal outcomes. We encourage those involved in care to attend consultations to support consistency and confidence.

 

Telehealth Consultations

Telehealth consultations are available via secure video or phone, providing timely clinical support and monitoring between visits—particularly valuable for regional or remote participants.

 

Education and Support
A key focus is empowering participants and carers through practical education on treatment techniques, infection prevention, skin integrity, and early identification of changes. This supports independence while reducing complications and hospital presentations.

Post-Consultation Support and Care Planning

With the consent of the patient, Wound Innovations provides them, and any other health professionals involved in the patient’s care, with a detailed Wound Management or Stoma Care Plan outlining our assessment findings, diagnosis, treatment and recommendations for follow up care.

Recommended next steps may include:

  • Scheduled review appointments
  • Evidence-based product recommendations such as dressings, skin care treatments, mattresses, nutritional supplements.
