Our approach is collaborative. We work closely with residents, their care teams, and facilities to develop individualised, evidence-based management plans tailored to each resident’s clinical condition, comorbidities, mobility, and overall care needs—ensuring they are practical and sustainable.
A key focus of our service is building the knowledge and confidence of facility staff. Through targeted education and support, we enable early identification, timely intervention, and improved day-to-day management, enhancing outcomes and reducing complications.

Onsite Facility Consultations
Our clinicians provide comprehensive onsite consultations, undertaking holistic assessments that consider medical history, risk factors, and current treatments. Additional assessments may be conducted where required to optimise care planning.
We emphasise hands-on learning, encouraging staff to participate in consultations to support real-time knowledge transfer, build confidence, and reinforce best practice.
Telehealth Consultations
To complement onsite services, telehealth consultations are available via secure video or phone, ensuring timely access to specialist input particularly for regional facilities or interim reviews.
Post-Consultation Support and Care Planning
Following each consultation, and with resident consent, a detailed Wound Management or Stoma Care Plan is provided. This includes assessment findings, treatment recommendations, product selection, and guidance for ongoing care and escalation.
Recommendations may include:
- Scheduled review appointments
- Evidence-based product recommendations such as dressings, skin care treatments, mattresses, nutritional supplements.
